Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
According to ANSI B30.11 Para 11-2.1.3: "Complete inspection of the crane
or monorail system
shall be performed at intervals as generally defined in Para 11.2.1.1-b.2
depending upon its
activity, severity of service and environment. These inspections shall
include
the requirements
of Para 11-2.1.2". The following subparagraphs apply:  d) Worn, cracked or
distorted parts; g)
Electrical apparatus.
Lone Star Auto Auctions, 2706 Slaton Hwy, Lubbock, Texas. On or about 21
February, 1997,
periodic (annual) inspections of underhung hoists had not been conducted.
The following
conditions were observed on the hoists in the mechanic's barn, one was in
limited operation,
three others were not being used. The make and model of each could not be
determined from
the floor level.
Instances: (applies specifically to the crane in the NW corner that was in
limited use, but also
to the cranes that were not used, but the possibility of use is apparent)
a)Wire rope was corroded.
b)Wire rope was permanently deformed by being "kinked" in numerous
locations.
c)The electrical cord connecting the control pendant to the unit has
degraded
to
where the internal conductors are coming through the outer insulation at
various
locations. The outer insulation and possible internal insulation has
degraded to a
point that when tested by an AC Sensor, the sensor indicated voltage one
foot
away from the cord.
These conditions exposed employees to being struck by material being
hoisted should the wire
rope break, or coming into contact with live electric circuits.
Among other methods, one feasible and accepted abatement method to correct
this hazard is to
implement a inspection program based on the recommendations of the
manufacturer and require
that a formal inspection be performed on the equipment at intervals not
greater than 1 year.
