Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: 5(a)(1): Fina Big Spring Refinery, I-20 & Refinery Rd, Big Spring TX. On or about 23 Jul 97, the employer did not maintain pressure and temperature gauges in an operable condition throughout the facility. Gauges that would alert employees working in specific hazardous areas of the refinery were pegged (indicating that over pressure or over temperature occurred), corroded or damaged. These gauges give maintenance personnel information while in the refinery and away from the main computer controls. This condition exposed employees to the hazards and injuries related to the over charging of lines or the sudden release of hydrocarbons and related materials. Instances: Note that the instances below represent a sampling, similar conditions with gauges exist throughout the facility. a) FCCU- 06PM-033 pressure gauge - pegged, broken seal b) FCCU- LV3323 - 3 small gauges rusted and exposed (glass/plastic covers missing) c) FCCU- LV3322 gauge rusted d) FCCU- LV2281, LV3324, 06PV-4489, LV3325 gauges damaged e) 011F-021 gauge damaged f) Alky unit- 8 ft behind 011D-032 - gauge pegged g) RLC-208, CFCV208 gauges pegged h) P106, P107 reflux gauges damaged i) EC2048A pressure gauge pegged j) VFC3058 gauges damaged k) 80FY2217 fuel gas gauge pegged l) 80PV4400 gauge w/o cover, exposed to environment m) 25FY2263 gauge defective
