5(a)(1):
Fina Big Spring Refinery, I-20 & Refinery Rd, Big Spring TX. On or about
23 Jul 97, the
employer did not maintain pressure and temperature gauges in an operable
condition throughout
the facility. Gauges that would alert employees working in specific
hazardous areas of the
refinery were pegged (indicating that over pressure or over temperature
occurred), corroded or
damaged. These gauges give maintenance personnel information while in the
refinery and away
from the main computer controls. This condition exposed employees to the
hazards and injuries
related to the over charging of lines or the sudden release of
hydrocarbons and related materials.
