Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
a)  The weight indicator diaphragm was mounted on the dead line at a
height of approximately
6.5 feet.  This object was not secured to the derrick or mast by a
separate safety line as
recommeded by Section 8.10.2 of the American Petroleum Institute
Recommended Practice 54-
92. Employees were thus not protected against the hazards of being struck
by this object, in the
event that it became detached from the dead line or the dead line became
slack.
Among other things, one feasible method of abating this hazard is to
secure the diaphragm to
the derrick or mast by a substantial safety chain.
