Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
Financial Equipment Corporation, 301 W. 3rd, Hereford.  For the period of
time up to and
including 9/1/99, employees were required to make adjustments to a vault
door assembly,
weighing approximately 4000 pounds, before final installation without any
form of retention,
which caused the assembly to become unstable, resulting in the assembly
falling forward,
pinning an employee between it and the concrete floor.  According to the
employer's written
procedures for an installation of this type, "adequate safety devices are
to be installed to prevent
door from tipping out of opening".  This exposed employees to injuries
associated with being
crushed by a vault door.
