in the facility without
wearing the safety belts provided.  This exposed employees to the hazards
of being struck /
crushed by the lift truck overhead protection device should the fork lift
be accidentally tipped
over and result in the driver being thrown out of the operator seat.
A feasible and acceptable abatement method to correct this hazard would
be:  Ensure that drivers
wear supplied safety belts anytime they operate an industrial fork lift
