Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: At the job-site, located at the GEO/ARCO 44 #2 well, 4 miles S.W. of Grady, TX 79749: An employee, operating a Hot-Oiler (for Basic Energy Services, Inc., formerly dba Sierra Well Services, Inc.), to assist a work-over rig (belonging to Key Energy Services, Inc.) in getting a well bore flow / circulation, was exposed to hazards created by non-operational temperature gauges, at the Hot-Oiler operator's station. Feasible and acceptable abatement methods to correct these hazards would be to: Replace and maintain at all times (in good operational condition) pressure gauges at / within plain view of the operator, to ensure that the maximum system pressures are not exceeded, anytime the Hot-Oiler is connected for charging / pressurizing at the well bore.
