were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:

On or about September 30, 2000, employees were exposed to the hazards
associated with
explosion as a result of working in the area of an P&G model 16WB, which
due to being
damaged was not repaired in accordance with procedures listed in ANSI/NB
23. The National
Board Inspection Code (1998), RC-1010 (scope) covers repairs and
alterations to pressure
retaining items. RC-1030 stipulates that organizations performing repairs
and alterations shall
be accredited. RE-1090 welding shall be performed in accordance with the
requirements
of the
original code of construction and the welders must be qualified to perform
such repairs.
The weld on the bottom of the abrasive welder was not made in accordance
with ANSI/NB 23
as evidenced by lack of documention of the repair, lack of a "R" stamp
near the weld, and the
appearance of the weld in general.
Employees were not protected from: explosions as a result of the
catastrophic failure of the
pressure vessel.
Feasible abatement methods include, but are not limited to:
a)Having the P&G abrasive blasting pot repaired at a facility that is
qualified to make such
repairs.
b)Removing the P&G abrasive blasting pot from service and rendering
unusable for further use
as a pressure vessel.e
