Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees. Employees were in self propelled extensible boom aerial lift without any fall arrest. Among feasible means of abatement would be to comply with the American National Standards Institute, ANSI A92.5-1992 Standard for Boom-supported Elevating Work Platforms and, 1.Have the employee wear fall arrest anytime that they are in the aerial lift. Over 30 feet above grade - Employees riding/working in extensible aerial lifts at the Warrenville jobsite were exposed to fall hazards. No fall protection was used. NO DOCUMENTATION AND CERTIFICATION REQUIRED FOR THIS ITEM.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free of recognized hazards that were likely to cause death or serious physical harm to employees. An employee was allowed to stand and work from the midrail of a extendible boom aerial lift. 15 feet above grade - An employee conducting precast erection at the Warrenville jobsite from an aerial lift was exposed to fall hazards. The employee was allowed to stand on the midrail of the lift basket to reach a greater working height. NO DOCUMENTATION REQUIREDATION
