Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in exposing them
to a fall of 4 stories on or about July 11, 1996, employees operating or
working near an open
freight elevator shaftway were exposed to falls of four stories. The
freight elevator interlock
switch was by passed to permit the operation of the elevator with the
landing door on the 4th
floor open.
a)14 Hall St., Brooklyn, N.Y. - The freight elevator in the front of
building # 1
was operated unsafely with the 4th floor landing door open and bypassed
intewrlocks; on or about July 12, 1996.
ABATEMENT NOTE: A feasible and acceptable abatement method, among others,
to correct
the hazard would be to follow ASME/ANSI A17.3-1993, Section 2.7.1. It is
not permitted to
install device or objects which could render hoistway mechanical locks and
electrical contacts
inoperative. The "bypass" material must be removed and employees
instructed in the dangers
of bypassing elevator interlocks.
