Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a)On or about November 26, 1997, at the Monadnock Construction site at 345
W
13thStreet, New York, N.Y., at the main elevator which was being used for
removal of demolition debris employees were exposed to fall hazards to 60
feet
into unguarded elevator shafts and being struck by the moving elevator due
to
broken hoistway door interlocks which allowed the doors to roll open freely
exposing the unguarded elevator shaft.
Among other methods, one feasible and acceptable abatement method to
correct this hazard  is
to: Provide working hoistway door interlocks as required for compliance
with industry consensus
standard ANSI Standard A17.3 1986 Safety Code for Existing Elevators,
Section
2.7.1,
Hoistway Door Locking Devices.
