General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the load slip off the hook: a)Buncher Dept., Heavy Buncher Line, on or about 4/25/97: One Coffing (Duff Nortan) model EC 2016.3 electric chain hoist one ton capacity SN ECI- C-218-NDO being used to place reels of wire in bunching machines had the hook throat latch missing from the load hook. b)Buncher Dept., Buncher Machines #1-10, on or about 5/6/97: One CM Value Star Model WL electric chain hoist, one ton capacity SN LW-208H6 being used to hoist spools of wire from buncher machines and replace with new spools. The hoist throat latch was damaged in that it would not prevent a load from slipping off the load hook accidentally. Abatement Notes: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install and/or repair hook throat latches, and follow the requirements of American National Standard for electric chain hoists (ANSI/ASME HST-IM-1989). c)Buncher Dept., Heavy Buncher Line, on or about 4/25/97: One below the hook lifting device used for lifting reels of wire and placing them in Bunching Machine was not tested and rated as to it's lifting capacity. d)Anealing Dept., Battery Charging Area, on or about 4/25/97: One below the hook lifting device (32 inches long angle iron spreader bar) used by employees to remove and replace batteries for walk behind lift truck was not tested and rated as to it's lifting capacity. Abatement Notes: Among other methods, one feasible and acceptable abatement method to correct this hazard is: prior to initial use the lifting device shall be tested by or under the direction of an appointed person and a written report furnished by such person confirming the load rating of the device per ANSI/ASME B30.20-1993; and conduct initial, frequent and periodic inspections of lifting devices by designated personnel for wear, deterioration or malfunction per ANSI/ASME B30.20-1993.
