Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, supporting structure or below-the-hook lifting devices fail: a)On or about 6/19/97, the following overhead hoists had inoperable upper limit switches, and/or the supporting structures for the hoists were underrated for what the hoist capacity was rated for and/or the rated load capacity for the supporting structure was unknown, including, but not limited to: 1)Dept. 857, Tool Room, CM Lodestar overhead hoist #26, upper limit switch was inoperable. 2)Dept. 837, CM Lodestar overhead hoist #3, upper limit switch was inoperable and the rated load capacity for the monorail system was unknown. 3)Dept. 839, CM Valustar overhead hoist #6 rated for 500 lbs. was mounted to a Gorbel jib rated for 250 lbs. 4)Dept. 842, CM Valustar overhead hoist #8 rated for one ton was mounted to a Spanco monorail rated for 1/2 ton. 5)Dept. 842, Column D-6, CM Valustar overhead hoist #9 was mounted to the ceiling joists by a chain. 6)Dept. 842, Column D-9, CM Valustar overhead hoist (no identification number) used to lift compressors rated for 1/2 ton was mounted to a Spanco track rated for 250 lbs. 7)Dept. 842, CM Valustar overhead hoist #13, the rated load capacity for the monorail system was unknown. 8)Dept. 842, CM Valustar overhead hoist #10, the rated load capacity on the jib was unknown. 9)Dept. 849, CM Lodestar overhead hoist #14, upper limit switch was inoperable. 10)Dept. 844, CM Valustar overhead hoist #18, the rated load capacity on the monorail system was unknown. 11)Dept. 844, CM Valustar overhead hoists #19 and #20, each rated for 1/2 ton, both were mounted to a monorail system rated for 1/2 ton.Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: 1.Adjust and/or repair upper limit switches to prevent the load block from passing the safe upper limit of travel. 2.Prior to installation and use, ensure the supporting structure (jib, monorail, bridge) is designed to withstand the loads and forces imposed by the hoist for the rated load. 3.Conduct frequent inspections and perform the required maintenance following these inspections in accordance with ANSI B30.16-1987, Chapter 16-2. b)On or about 6/12-19/97, the following below-the-hook lifting devices had no rated load markings, no operational test performed and no initial, frequent or periodic inspections performed, including, but not limited to: 1)Dept. 857, Column E-13, Tool Room, two below-the-hook lifting devices used for lifting dies. 2)Dept. 849, LHP Motor Area, below-the-hook lifting devices, identifiable as a ring with two "J-type" hooks. 3)Dept. 842, Column D-9, below-the-hook lifting devices for lifting compressors. 4)Dept. 837, Column J-9, below-the-hook lifting devices, identifiable as a ring with hook, attached to a nylon sling for lifting compressors. 5)Dept. 846, below-the-hook lifting device "spreader beam" attached to a Demag overhead hoist #17. Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: 1.Prior to initial use, all new, altered, modified or repaired lifting devices shall be inspected by a designated person and an operational test performed to verify compliance with ASME B30.20-1993, Chapter 20-1. 2.Mark the rated load of the lifting device on the main structure or equivalent. 3.Conduct frequent and periodic inspections of the lifting devices in accordance with ASME B30.20-1993, Chapter 20-1. Abatement certification is required for this item, instances a-7, a-8, a-10 and instances b-1 through 5.
