Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist or below-the- hook lifting devices fail: a)Pre-Make Ready, on or about 10/2/97, Coffing 1/2-ton overhead hoist, upper limit switch was inoperable, exposing employees to the hazard of being struck by the load. Among other methods, some feasible and acceptable abatement methods to correct this hazard are to: 1.Adjust and/or repair the upper limit switch to prevent the load block from passing the safe upper limit of travel. 2.Conduct frequent inspections of the hoists to identify any defects and/or damage in accordance with ANSI B30.16-1987, Chapter 16-2. b)Pre-Make Ready, Press Room and Battery Charging Area, on or about 10/6/97, no inspections were conducted on five below-the-hook lifting devices. Two of those devices (spreader beams) located in Pre-Make Ready were not provided with rated load markings on the main structure. Employees exposed to the hazard of being struck by the load. Among other methods, some feasible and acceptable abatement methods to correct these hazards are to: 1.Mark the rated load of the lifting device on the main structure. 2.Conduct frequent and periodic inspections of the lifting devices in accordance with ASME B30.20-1993, Chapter 20-1. Abatement documentation is required for instance b.
