employees were exposed to THE HAZARD OF BEING STRUCK BY THE LOAD
SHOULD THE LOAD SLIP OFF THE HOOK:
a)West side of Building #1, on or about 12/18/97: A CM
Loadstar Chain hoist, serial number L-167 H L, 1 ton
capacity, had the hook throat latch missing from the load
hook.
ABATEMENT NOTES:
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO:
1.INSTALL AND/OR REPAIR HOOK THROAT LATCHES.
2.FOLLOW THE REQUIREMENTS OF AMERICAN
NATIONAL STANDARD FOR ELECTRIC CHAIN HOISTS
(ANSI B30.16-1973)
