Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling loads and/ or lifting equipment: a)Foundry Area, on or about 2/24/98: All of the below the hook lifting devices used for suspending molds were not inspected or tested. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to correct this hazard is to: Conduct initial, frequent, and periodic inspections of the lifting devices by designated personnel for wear, deterioration or malfunction as per ASME B30.20- 1993, chapters 20-1.3 and 20-1.4. b)Foundry Area, on or about 2/24/98: All of the material hoists (One Pneumatic Chain Hoist, Two Electric Chain Hoists, and Two Manual Chain Hoists) were not inspected. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to correct this hazard is to: Conduct initial, frequent, and periodic inspections of the hoists and support structures for wear, deterioration or malfunction as per ASME/ANSI B30.16-1987, chapter 16-2. c)Foundry Area, on or about 2/24/98: All of the material hoists (One Pneumatic Chain Hoist, Two Electric Chain Hoists, and Two Manual Chain Hoists) were not provided with any load ratings. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to correct this hazard is to: Provide load ratings for the hoists, hooks, chains, and support structures as per ASME/ANSI B30.16-1987, chapter 16-1. d)Foundry Area, on or about 2/24/98: One Pneumatic Chain Hoist, and Two Manual Chain Hoists were not provided with hook latches. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to correct this hazard is to: Provided hook latches for hooks as per ASME/ANSI B30.16-1987, chapter 16-1.2.9.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of receiving various injuries from the lack of enforcement of the wearing of seatbelts: a)First Floor, on or about 2/24/98: Employees were not wearing seat belts provided on the fork lift truck. Employees exposed to the hazard of being struck by, or thrown from the fork truck in an accident. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to correct this hazard is to require and enforce the use of the seat belts that are provided on the fork truck and as required by ASME B56.1-1993, chapter 5.3.19. Abatement documentation is required for this item.
