devices
used for suspending molds were not inspected or tested.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable method to correct this
hazard is
to:
Conduct initial, frequent, and periodic inspections of the lifting devices
by
designated personnel for wear, deterioration or malfunction as per ASME
B30.20-
1993, chapters 20-1.3 and 20-1.4.
b)Foundry Area, on or about 2/24/98:  All of the material hoists (One
Pneumatic Chain Hoist, Two Electric Chain Hoists, and Two Manual Chain
Hoists) were not inspected.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable method to correct this
hazard is
to:
Conduct initial, frequent, and periodic inspections of the hoists and
support
structures for wear, deterioration or malfunction as per ASME/ANSI
B30.16-1987,
chapter 16-2.
c)Foundry Area, on or about 2/24/98:  All of the material hoists (One
Pneumatic Chain Hoist, Two Electric Chain Hoists, and Two Manual Chain
Hoists) were not provided with any load ratings.
ABATEMENT NOTES:
Among other methods, one feasible and acceptable method to correct this
hazard is
to:
Provide load ratings for the hoists, hooks, chains, and support structures
as per
ASME/ANSI B30.16-1987, chapter 16-1.
d)Foundry Area, on or about 2/24/98:  One Pneumatic Chain Hoist, and Two
Manual Chain Hoists were not provided with hook latches.

fork truck
and as required by ASME B56.1-1993, chapter 5.3.19.
