Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist and its components fail: a)On or about 3/3/98, the following overhead hoists had safety latches missing from the hooks, and/or the upper limit switch was not functioning and inspections had not been conducted, including, but not limited to: 1)In the Shop, Coffing two ton overhead hoist near the Amada press brake had the safety latch missing on the hook and no inspections performed. 2)In the Shop, two Quik Lift one ton overhead hoists near the Verson press brake, one hoist had the safety latch missing on the hook and on the other hoist, the latch was broken, and no inspections performed. 3)In the Shop, west side, a Coffing three ton overhead hoist had no inspections performed. 4)In the Shop, Boiler Assembly Area, two Coffing two ton overhead hoists had the safety latches missing from the hooks, no inspections conducted and on one of the hoists, the upper limit switch was not functioning. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Install safety latches on the lifting hooks. 2.Repair the upper limit switch. 3.Conduct frequent and periodic inspections in accordance with ANSI B30.16-1987, Chapter 16-2. Abatement certification is required for these items.
