Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK BY MOBILE EQUIPMENT WHICH HAS AN OBSTRUCTED VIEW TO THE REAR: a)Mobile Equipment, on or about 6/24/98: Truck #75, 1990 International Model 4700 mobile recycling vehicle reverse signal alarm was inoperative. Employees exposed to a struck by hazard. b)Mobile Equipment, on or about 6/24/98: Truck #537, 1989 Crane Carrier side loading mobile collecting vehicle reverse signal alarm was inoperative. Employees exposed to a struck by hazard. c)Mobile Equipment, on or about 6/24/98: Truck #37, 1989 Mack Model DM rear loading mobile collecting vehicle reverse signal alarm was inoperative. Employees exposed to a struck by hazard. ABATEMENT NOTES: 1.Repair or replace defective reverse signal alarms. 2.Follow the requirements of American National Standards for Refuse Collecting and Compacting Equiptment(ANSI Z245.1-1984), Section 7.1.14.2.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK BY THE LOAD SHOULD THE LOAD SLIP OFF THE HOOK: a)Shop I, on or about 6/11/98: Shaw Box manually operated chain hoist's #1,2,3 & 4 all had load hook throat latches missing. Employees exposed to struck by hazard. b)Shop II, on or about 6/12/98: Robbins & Myers Overhead Hoist throat latch missing from load hook. Employees exposed to struck by hazard. ABATEMENT NOTES: 1.Install and/or repair hook throat latches. 2.Follow the requirements of American National Standard for Manually Operated Chain Hoists (ANSI/ASME HST-2M-1989). c)Shop I, on or about 6/11/98: (Two) 1 & 1/2 ton capacity Com-A-Longs, had load hook throat latches missing. Employees exposed to struck by hazard. ABATEMENT NOTES: 1)Install and/or repair hook throat latches. 2)Follow the requirements of American National Standards for Manually Lever Operated Chain Hoists( ANSI/ASME HST-3M-1991). d)Shop I, on or about 6/11/98: manually operated chain hoists designated as #'s 2A,1B,3B & 4A were not marked as to their rated load. Employees exposed to struck by hazard. ABATEMENT NOTES: 1)Follow the requirements of American National Standard for Manually Operated Chain Hoists(ANSI/ASME HST-2M-1989), Section 4.2.1. Abatement certification must be submitted for this item.
