Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)Hallway by warehouse, on or about 06/24/98: Below the hook lifting device used to remove and install 2295 lb forklift batteries was not inspected or tested. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to abate this hazard is to: Conduct initial, frequent and periodic inspections of lifting devices by designated personnel for wear, deterioration or malfunction as per ASME B30.20-1993 b)Hallway by warehouse, on or about 06/24/98: One of four material hoists was not provided with hook latches. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to abate this hazard is to: Provide hook latch for hook as per ASME/ANSI B30.16-1987. c)In the workplace, on or about 06/24/98: Four material hoists were not inspected on regular basis. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to abate this hazard is to: Conduct initial, frequent and periodic inspections of the hoists and support structures for wear, deterioration or malfunction as per ASME/ANSI B30.16-1987. Abatement documentation is required for this item.

ABATEMENT NOTES: Among other methods, one feasible and acceptable abatement method to correct this hazard is to apply physical guards as per ASME B20.1a-1991 to prevent employee contact. Abatement certification is required for this item.
