employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF FALLING OFF THE EQUIPMENT.
a)Job Site, T/O Trenton Highway Garage, on or about 11/12/98: One employee
standing on a wooden pallet mounted on the forks of a Caterpillar Rough
Terrain fork lift, no safety platform provided. Employee was in a raised
position using a cutting torch doing structural steel erection
approximately
twenty-three feet above ground level.
ABATEMENT NOTES:
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO:
1.Use a safety platform secured to the lifting carriage and/or forks.
2.Follow the requirements of American National Standards B56.6-1992, Safety
Standard for Rough Terrain Forklift Trucks, Section 5.15, Elevating
Personnel.
b)Job Site, T/O Trenton Highway Garage, on or about 12/30/98: One employee
standing in a bucket attachment of a Caterpillar model TH-83, rough terrain
fork lift approx. twelve feet above the ground level. Employee was working
on metal roofing.
ABATEMENT NOTES:
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO:
1.Use a safety platform secured to the lifting carriage and/or forks.

a)Job Site, T/O Trenton Highway Garage, on or about 12/07/98: Employee's
exposed to a struck-by hazard because the operator of a Caterpillar Rough
Terrain fork lift had left the equipment's operator position with forks
and/or
forks with a load in an elevated position. Forks and/or forks with a load
were
elevated approx.12 feet.
ABATEMENT NOTES:
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO:
1.Before leaving the operator's position fully lower the load-engaging
means.
2.Follow the requirements of American National Standards B56.6-1992, Safety
Standard for Rough Terrain Forklift Trucks, Section 6.2.12, Operating
Safety Rules and Practices (General).
