P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of being struck by material handling equipment: or in the alternative 29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: a.Foundry Roadway at the west side of the foundry building - There were no traffic signs warning of the simultaneous use of the roadway pavement by employees as pedestrians where the Grinnell Corporation 1986 red Ford 8000 dump truck was used in reverse gear and the 1988 white Ford L8000 dump truck was also used, on or about 11/12/97. Among other methods, one feasible and acceptable abatement method to correct this hazard is to display traffic signs warning drivers to watch for workers and warning that pedestrians may also be using the roadway; that the speed limit is only five miles an hour; that it is a high noise area; and a back-up alarm or signal person must be used while backing trucks along the roadway.

P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of being struck by material handling equipment used in reverse gear without a reverse signal alarm or an employee who signalled it was safe to do so: a.Foundry Roadway by the Bag House Dust Collector near Column 23 - The Grinnell Corporation 1986 red Ford 8000 dump truck was used in reverse gear along the roadway pavement at the west side of the foundry building, on or about 11/12/97. Among other methods, one feasible and acceptable abatement method to correct this hazard is to equip each dump truck with an audio back-up warning system.
