1970: The employer
did not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of been struck-by or crushed by:
a) Loading dock; Employees were observed operating two(2) each Nissan
industrial forklift
trucks and were not wearing seat belts and were thereby exposed to
struck-by/crushed-by
hazards, should the forklift(s) accidently tip over and cause fatal upper
body
injuries if they were thrown out of the operator's seat on or about
3-26-98.
One feasible and acceptable abatement method to correct this hazard is to
require industrial
forklift truck operators to wear seat belts. For additional assistance,

B56.1a-1989 Addenda to ASME/ANSI B56.1-1988 and ASME/ANSI B56.1-1993.
