industrial forklift
truck without wearing the seatbelts that were provided.  this was exposing
the operator to struck-
by/crushed-by hazards, should the forklift be accidently tipped over, that
could result in fatal
injuries when their upper body parts were struck/crushed by the lift truck
overhead protection
device as they were thrown out of the operator seat, on or about 9/20/99.
Other feasible and acceptable abatement methods to correct this hazard
would be to ensure that
employees wear seatbelts while operating industrial trucks.
