P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of
1970: The employer
did not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazards of being struck by a loose whipping
hose and/or contact
with hot black liquor:
a.  Black Liquor process area located to the southeast of the Turpentine
process area: The types
of hoses used by employees to pump hot black liquor from the Strong Black
Liquor Filtrate Tank
(M-150043) to Foam Tank (M1500580) were not appropriate for handling that
material.  Nor
was the hose assembly properly connected to the Foam Tank (M150058) to
prevent it from
coming apart, on or about 12/17/99.
Among other methods, one feasible and acceptable method to correct this
hazard is to:
1. Select and use hose assemblies that are compatible with the use
application
and in accordance
with the manufacturer's recommendations.
2. Inspect hose assemblies prior to each use.  Repair or replace missing
or damaged hose
assembly components.
3. Verify that hose assemblies are properly connected in accordance with
the manufacturer's
recommendations prior to performing transfer operations.

manufacturers/suppliers regarding
recommendations for particular use applications.
