P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of falling from an elevated platform. a) Warehouse - A forklift truck with an unapproved platform was used for lifting personnel to perform work, on or about 04/11/00. b) Warehouse - Employees were allowed to ride on the platform, while this equipment was traveling, on or about 04/11/00. Among other methods, one feasible and acceptable abatement method to correct this hazard is: To provide an approved platform designed for this purpose, securely attached to the forks and to prohibit the practice of riding on this platform while the forklift truck is traveling. For additional assistance please refer to ASME B56.1-1993 (Revision of AMSE/ANSI B56.1- 1988) Safety Standard for Low Lift and High Lift Trucks, Part 11, Section 4.17.
