a)At Thames Water of P.R. on December 26 2001, employees performed a
non-routine
task of loading and unloading liquid chlorine ton containers from a
trailer. There was no
dock area to park the trailer and for that reason the forklift could not
get inside the
trailer. Two employees went inside the trailer to move manually 15 liquid
chlorine ton
containers that each one weight from 2,000 to 2,200 pounds approximately,
they rolled
them to the exit of the trailer. Them a forklift trademark Yale, model
ERP040TFN36SF082 serial # B807N030370 was used to carry the ton containers
to the
storage area at the Chlorine Building. The employees were exposed to back,
hand and
foot injuries, and chlorine intoxication or death in case of accidental
release of the
product.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is:
1.Always receive the containers in a flat-bed trailer, because the
facilities were designed
to use that type of equipment to loading and unloading the ton containers.
2.Provide a dock area, to use the forklift with the appropriate
attachments to go inside the
trailer for unloading and loading the chlorine ton containers.
a)En Thames Water de Puerto Rico el 26 de diciembre de 2001, los empleados
realizaban
una tarea no rutinaria de cargar y descargar contenedores de cloro liquido
desde un
vagon. No habia un muelle de carga y descarga para estacionar el vagon y
por tal razon
el montacargas no podia entrar dentro del vagon. Dos empleados entraron al
vagon para
cargar y descargar 15 contenedores de cloro liquido, cada uno de los
contenedores pesa
de 2,000 a 2,200 lbs aproximadamente, los empleados rodaban los
contenedores hasta
la salida del vagon, entonces un forklift marca Yale, modelo
ERPO4OTFN36SFO82,
# de serie B8O7NO3O37O fue usado para llevar los contenedores al area de
almacenamiento en el Edificio de Cloro. Los empleados fueron expuestos a
lesiones de
espalda, pies y manos e intoxicacion por cloro o muerte en caso de un
escape accidental.
Entre otros metodos ,un metodo viable y aceptable de correccion para
corregir este
riesgo es:
1.Siempre recibir los contenedores en un remolque de plataforma, porque la
facilidades de
la compania estan disenadas para descargar y cargar los cilindros de dicha
forma.
2.Proveer un muelle de carga y descarga, para usar el montacargas con los
aditamentos
adecuados para entrar dentro del vagon para decargar y cargar los
contenedores.

trained to perform
this specific task.
Employees were exposed to body injuries and death.
Among other methods, one feasible and acceptable abatement method to

hazard is:
1.Evaluate the tasks and establish and develop a written safety procedure
for this task and
train the employees.

una tarea no rutinaria de descargar y cargar un vagon de 15 contenedores
de cloro
liquido. Un procedimiento escrito de seguridad no fue establecido y los
empleados no
fueron adiestrados para realizar esta tarea en especifico.
Entre otros metodos, un metodo viable y aceptable de correccion para
corregir este
riesgo es:
1.Evaluar la tarea, hacer un procedimiento escrito de seguridad para esta
tarea y adiestrar
a los empleados.
