causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
a)Asphyxiation, chemical poisoning and/or death from the hazards of oxygen
deficiency and/or dangerous accumulation of toxic gases (such as carbon
monoxide) while entering manhole to do drain water and arrange electrical
cable work.
These hazards resulted from a Company lack of knowledge on confined space
regulations, on confine space identification and evaluation procedures, on
confine space hazard recognition and a lack of a confined space entry
program
for this project.
The American National Standards Institute standard (ANSI Z117.1-1989)
"Safety Requirements for Confined Spaces" contain acceptable methods of
compliance which include the following:
1.Confined Space Survey shall be conducted by a qualified person.
2.Hazard Identification
3.Hazard Evaluation by a qualified person
4.Confined Space Classification
5.Hazard Re-Evaluation
6.Atmospheric Testing
7.Ventilation
8.Safeguards (ie. enter and exit means, retrieval equipment, fall
protection)
9.Training (Attendants and occupants)
10.Emergency Response Plan
a)Asfixia, envenenamiento quimico y/o muerte por riesgos de deficiencia de
oxigeno y/o acumulacion peligrosa de gases toxicos (como monoxido de
carbono) mientras entraban al registro para realizar trabajos de sacar
agua
y
acomodar cables electricos.
Estos riesgos son el resultado del desconocimiento en la compania sobre las
normas aplicables a espacios confinados, los procedimientos de
identificacion y
evaluacion de espacios confinados, en el reconocimiento de riesgos
asociados a
espacios confinados y la ausencia de un programa para entrada a espacios
confinados para este proyecto.La norma del American National Standard
Institute (ANSI Z117.1 - 1989)
"Requisitos de Seguridad para Espacios Confinados" contiene metodos de
cumplimiento aceptables que incluyen lo siguiente:
1.Estudio para identificar Espacios Confinados a ser conducido por una
persona calificada.
2.Identificacion de riesgos
3.Evaluacion de riesgos por una persona competente.
4.Clasificacion de espacios Confinados
5.Re-Evaluacion de riesgos.
6.Muestreos
7.Ventilacion
8.Salvaguardas (ej. medios de salida y entrada, equipo de rescate,
proteccion contra caidas)
9.Adiestramiento (asistentes, ocupantes)
10.Plan de Respuesta de Emergencia."
