(a) Fieldstown Rd. & Hwy 31, Gardendale Al. An employee was put
down in a 12 foot deep excavation in the bucket of a Komatsu PC
2000 backhoe, exposing him to the hazard of being crushed between
the bucket and the excavation wall or falling from the bucket.
"Among other methods, one feasible and exceptable method to correct
this hazard is to provide a ladder.
