Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a struck-by hazard due to wire rope failure:
a) M.E.R.F., Small Engine Repair Shop - Kinked, and abraded wires were
observed
on the running rope of the three ton electric hoist.  The hoist, mounted
on a jib, was
used to lift the ends of the buggies onto blocks or jack stands for repair.
b) Saw Shop - Permanent distortions (dog leg) were observed in the running
rope of
the 1/2 ton hoist in the shipping/receiving area.  The hoist was used to
transport saw
blades to and from trucks at the dock.
ABATEMENT NOTE:  Among others, one feasible and acceptable method to abate
this
hazard is to replace the wire rope on the hoists.

employees in that
employees were exposed to hazards due to wire rope failure:
a) Auxiliary Shipping Yard; 33 Run - The guide roll on the feed side of
the T-bar
rotary was suspended from a 1/2" wire rope.  The rope was terminated using
a single
wire rope clip on the roll side as well as on the lift cylinder side.  A
single clip does
not properly terminate the 1/2"  wire rope, thus exposing employees to
injury in the
event of clip failure.
b) 2 Battery Door Repair - Wire rope clips used to terminate the hoist
line of the
north crane, west block were applied backward.
c) 10B Conveyor Tube - Two of the five wire rope U-bolt clips used to
terminate the
end of the 1" cable at the A coal tank were applied backward.
ABATEMENT NOTE:  Among others, one feasible and acceptable method to abate
this
hazard is to apply clips as recommended by the manufacturer.

were exposed to the hazard of a falling load due to improper installation
of the wedge and socket
(becket) fitting(s) on the load line:
a)  420 Saucon Machine Shop - The hoist line on crane #624 was connected
with two (2)
wedge and socket fittings that were installed backwards and the tail of
the rope was
secured with tape thereby exposing the employees to the potential of a
falling load.
Among other methods, one feasible and acceptable method to correct these
hazards is to install
the wedge and socket fitting with the rope in a straight line with the
fitting and secure the tail
with a wire rope clip applied to the tail of the dead end only.
