Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing hazard from a falling load:
a)  Saw Area - The hand operated chain hoist is used to lift slabs of
un-cut marble. These
slabs weigh a maximum weight of 1500 pounds and are hoisted four (4) feet
off the floor
to place them on the cutting table. The chain hoist, the below the hook
lifting device and
the chain sling had the following deficiencies:
1.  The spreader bar was a piece of shelving angle iron that was clamped
to the
slab with three (3) C clamps.
2.  The chain sling was attached to the spreader bar at either end with a
bolt
through the chain link and then through the hole in the end of the beam.
3.  The chain sling, used for overhead lifting, was a mild steel and not
alloy
steel.
4.  The chain sling was not long enough so an "S" type connector was used
to
lengthen the chain.
5.  The single leg chain sling was run over the hoist hook.
6.  The hoist hook was connected to the hoist chain with a common steel
bolt and
the connection required a steel pin.
7.  The chain hoist hung on the mechanical splice of the eyes on a 1/2
inch wire
rope sling.
8.  The 1/2 inch wire rope sling had a piece of channel iron threaded
through the
eyes of the sling exposing the sling to sharp corners.
9.  The hoist was suspended from two roof joist and no engineering study
had
been performed to show these joist were strong enough to support the
maximum
intended load of 1500 pounds.
10.  The end of the hoist chain was connected to the hoist with a common
steel
bolt and not the required steel pin.
ABATEMENT NOTE:  The employer should install the hoist in accordance with
American
National Standards Institute (ANSI) B30.16 Overhead Hoists, ANSI B30.20
Below the Hook
Lifting Devices and ANSI B30.9 Slings.
