employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck by objects due to
failure of wire rope
on the underhung cranes.
a) Shop Area; North Crane, West Hoist -  The READING crane 5 ton hoist was
operated with a running rope having a dog leg distortion and a sheave
showing
corrugations from the wire rope.
b) Shop Area; South Crane, West Hoist - The READING crane 5 ton hoist was
operated with a running rope which was crushed and flattened in at least
two places.
ABATEMENT NOTE: Among others, one feasible and acceptable method to abate
this
hazard is to replace the ropes and sheave on the hoists and to initiate a
program if inspections
on the cranes in accordance with the provisions of American National
Standards Institute
(ANSI) B30.11 Monorail Systems and Underhung Cranes, latest edition.
Abatement certification required within 10 days after abatement date.
The certification
shall include a statement that abatement is complete, date and method of
abatement,
and states employees and their representatives were informed of this
abatement.
