Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to defective hoists: a) Workplace, Bay 1 & Bay 2: The underhung cranes/hoists in the facility were not kept in good repair in that the following deficiencies were noted: 1. Wright Speedway: Running rope showed excessive wear and broken wires. 2. Wright Speedway: Non current carrying metallic parts of the pendent were energized with AC voltage. 3. Wright Speedway: Strain relief device of the pendent was supported with tape. 4. Yale: Strain relief of the pendant was inadequately supported with wire. 5. Yale: Knockout hole in the pendant box was inadequately closed with clear tape. Among other methods, one feasible and accepted abatement method to correct these hazards is to develop a preventative maintenance program which incorporates all recommendations outlined in the manufacturer's manuals, ANSI B30.16, Overhead Hoists and ANSI B30.11, Monorail Systems and Underhung Cranes, with written, signed and dated inspection reports. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
