Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling hoist parts and/or falling loads: a) Slitter Area - The #2 Yale 1/2 ton underhung hoist was stressed while performing lifts when the load weight was unknown and performing side lifts/pulls creating a failure on or about June 13, 1998. Among other methods, one feasible and acceptable methods to correct this hazard is: 1.To develop a preventative maintenance program which incorporates all recommendations outlined in the manufacturers manuals and applicable ANSI standards. 2.Review safe crane use and proper lifting practices with all affected employees. 3.Develop a system to weigh and mark the product rolls before they arrive in the slitting area for use. Abatement certification required within 10 days after abatement date. Certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
