Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees; in that employees were exposed to the hazard of accidental activation of the press when hand feeding metal bars into the dies of the mechanical press brake: (a)On or about 5/7/99 in the Press Room; employees hand feeding the Dreis and Krump #204 press brake, which did not have a guard on the foot pedal to prevent accidental activation. (b)On or about 5/7/99 in the Press Room; employees hand feeding the Gilbert #45 press brake, which did not have a guard on the foot pedal to prevent accidental activation. ANSI Standard B11.3-1982, paragraph 4.2.4.1.5 states "a means shall be provided to inhibit accidental activation" of a foot-treadle bar. Among other methods, one feasible and acceptable abatement method to correct this hazard is to put a cover guard over the foot-treadle bar. NO ABATEMENT DOCUMENTATION REQUIRED
