Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)  Employees were exposed to carbon dioxide used for refrigeration in
Blast Areas 5
and 6.  Carbon Dioxide monitors, used by the employer to monitor carbon
dioxide and
to provide a warning signal in the event an emergency evacuation was
needed, were not
maintained.  Employer did not have an effective means to ensure
calibration and
maintenance of carbon dioxide monitors used to establish carbon dioxide
levels during
the work shift or during a release.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to establish a calibration and maintenance program according to
manufacturer
