a.On or about October 16, 1997, the tiebacks on the suspension scaffold
went from the 2nd floor up and across the edge of the 4th floor to an
anchor point, creating a chafing point on the tieback.
One feasible and acceptable method to correct the hazard is to provide
aerial lifts for
employees to use in this area of the building.
