Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health
Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees, in that employees were exposed to hazard of falls at the
worksite known as Free
States Manufactured Homes, 500 Eastview Drive, Laurel, MS, where:
(a)Employees were exposed to the hazard of falling platform/catwalks
supported by
hoists when employees walked and worked approximately 7 feet above floor
level.  The
approximately 90 feet long unguarded platforms/catwalks positioned
alongside the homes
had one brace in the middle and fabricated platforms welded to the
platform ends were
placed unsecured onto a walking working surface.  Additional unguarded
platforms were
placed across the ends of the homes unsecured to the catwalks, all
supported by Coffing
hoists.  (Reference American National Standards Institute (ANSI)

Pub. Part No. EC680-5, Rule 31 "Do not use hoist to lift, support, or

(b)  Employees without fall protection worked on roof areas installing
decking, felt, and
metal roofing materials exposed to falling approximately 13 feet to the
concrete floor.
Among other methods one feasible and acceptable abatement method to
correct this hazard is
to prohibit employees from working on hoisted loads, install underbracing
to support the
intended loads on the work platforms, and use a fall arrest system for
work performed on roof
areas.

employees, in that employees were exposed to hazard of being struck-by
and/or crushed-by loads
at the worksite known as Free States Manufactured Homes, 500 Eastview
Drive, Laurel, MS,
where:
(a)  Employees were allowed to stand, reach and work underneath hoisted
loads, i.e.,
work platforms/catwalks along the sides and ends. (Reference ANSI
B30.16-1973,
Overhead Hoists, Chapter 16-2, Section 16-2.3.2.3(f) and Coffing Hoist
Operating &
Maintenance Instruction, Pub. Part No. EC680-5, Rule 32 "Do Not lift loads
over
people".)
(b)  Rated load tests to confirm the load rating of the monorail system
with Coffing Hoist
units were not available.  (Reference American National Standards
Institute B30.11-1973,
Monorail Systems and Underhung Cranes, Chapter 11-2, Section 11-2.2.2(a)).
(c)  The lifting devices used with the Coffing hoists did not have the
rated
load marked
on the lifter, nor was a nameplate or other permanent marking displaying
the
manufacturer's name, serial number, lifter weight, and rated load affixed
to the lifters
which are used to move walls, support and move roofs, and support floors
at the
electrical wiring station where support stands were not load rated.

(d)  Roofs supported by lifting devices attached to Coffing hoists were
left unattended
suspended approximatley 12 feet above the floor and no precautions were
taken to
prohibit employees walking underneath the load.  (Reference ANSI
B30.16-1973,
Chapter 16-2, Section 16-2.3.2.3(j)).
Among other methods one feasible and acceptable abatement method to
correct this hazard is
to prohibit employees from working on or reaching under hoisted loads,
install underbracing to
support the intended load on the work platforms, perform load tests and
maintain records of the
monorail system, and affix identifying information to the lifting devices.
