Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  the
employer did not furnish
employment and a place of employment which are free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that:
a) the personnel platform was not secured to the forks of the lift truck,
the worker
on the platform was not protected from contact with the moving mechanism on
the mast, and the worker was not protected with a complete guardrail
system.
b) the pendant control buttons on the hoist at the C.A. Cray milling
machine in
the fabrication shop, adjacent to the loading dock, were not legibly
identified.
c) the pendant control buttons on the Acco Vacuum Lift in the fabrication
shop
were not legibly identified.
