Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation injuries from unintended activation. a) Shop area: the foot pedal on the 15 ton Chicago press break, was not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedal. b) Shop area: the foot pedal on the Chicago press break, No.VT21, was not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedal. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED Among other methods, one feasible and acceptable abatement method to correct this hazard is to guard the foot treadle.
