a. An employee working in a Genie S-125 aerial lift adjacent to Crane #19
at the Wando
Terminal, climbed over the guardrails of the aerial lift and out of the
aerial lift and over the
guardrails of the stair railing and onto the steps of the gantry crane and
was exposed  to a
fall potential of approximately 40 ft. to the concrete dock below.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to walk up the provided and sufficiently guarded steps of the crane.
b. An employee riding in the Genie S-125 aerial lift adjacent to Crane #19
at the Wando
Terminal, had a frayed lanyard attached to the employee's body harness.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to follow the ANSI A10.14 (1991) concensus standard and implement a
system of
inspecting the personal fall protection equipment daily, and before each
use, this would
include  discarding and replacing any worn or defective equipment as
needed .eeded
