Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to falling and/or crushing injuries associated with unexpected activation (launching) of an amusement ride. a) Farside employee(s) were exposed to the hazards associated with unexpected activation of the Sidewinder amusement ride in that on and prior to August 2, 1997, the Stationside control panel was equipped with a "by-pass" switch which disabled safety features designed and built by the manufacturer to prevent unexpected activation (launching) of the train from the Farside station. Abatement Notes; Among others, one feasible abatement method to eliminate this hazard would be to remove the "by-pass" switch and follow written methods and procedures contained in the manufacturers Owners Manual. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazards of falling and/or crushing injures resulting from the unexpected start-up of the amusement ride due to the inability to communicate between operators. a) On and prior to August 2, 1997, employees operating the Farside position of the Sidewinder amusement ride were exposed to the hazards of falling and/or crushing injuries due to the inability to communicate between the Mainside and Farside work stations. Abatement Notes: Among others, one effective way to abate this hazard would be to repair the phone system between the Mainside and Farside stations. Abatement Note: Abatement certification is not required for this item.
