Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard associated with an improperly installed walkway: a.)Aboard the Zidell Barge, employees erecting the crane boom used a walkway which had not been installed according to the manufacturer's instructions, on or about 4 December 1998. Among other methods, one feasible and acceptable method to eliminate this hazard would be to have manufacturer's instructions available during the assembly and/or installation of equipment and accessories, and ensure those instructions are followed.
