AS 18.60.075(a)(4):  An employer shall do everything necessary to protect
the
life, health and safety of employees including, but not limited to
furnishing to
each of his employees employment and a place of employment which are free
from
recognized hazards which, in the opinion of the commissioner, are causing
or are
likely to cause death or serious physical harm to his employees.
EXAMPLE:  The employer failed to provide employees with a workplace free
from
recognized hazards, in that industry recognized requirements for ammonia
refrigeration systems are not being meet.  These requirements are defined
in the
American National Standards/American Society of Heating, Refrigeration,
and Air-
Conditioning Engineers standard (ANSI/ASHRAE 15-1994).
The ANSI/ASHRAE 15-1994 regulation item 8.14(i), was not being meet in
that the
controls to stop the refrigerant compressors and start the refrigeration
machine
room ventilation system was not installed and located immediately outside
the
machinery room.
Regulation item 11.2.1, was not being meet in that there was no legible
permanent
sign indicating the name and address of the installer, the refrigerant
number and
amount, the lubricant identity and amount, and the field test pressure
applied.
Regulation item 11.2.2, was not being meet in that there were no signs
identifying the valves or switches for controlling the refrigerant flow,
the
ventilation, the refrigeration compressors and the kind of refrigerant
contained
in exposed piping outside the machinery room.
Regulation item 11.8, was not being meet in that there was no schematic
drawing
or panel giving directions for the operation of the system which shall be
conveniently located to the operators.  Precautions to take in case of a
breakdown or refrigerant leak shall be displayed on a conspicuous card
located
as near as possible to the refrigerant compressor.  These precautions shall
address instructions for shutting down system in case of emergency; the
name,
address, and day and night telephone numbers for obtaining service; and
names,
addresses, and telephone numbers of all corporate, local, state, and
federal
agencies to be contacted as required in the event of a reportable
incident.
Also, there was no emergency shutdown procedures posted outside the
machine room,
immediately adjacent to each door.
