Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries:
a)  Employees of The Electron Corporation were exposed to crushing
injuries while transporting
and positioning castings using the Lift-Tech Budgit Series 6000 Man Guard
Air Hoist.  Sling
hooks used to attach the castings to the hoist were not positioned so that
the casting weight was
supported by the saddle of the hook rather than the hook tips, and
employees had not received
adequate training in relation to proper placement of loads onto slings and
sling hooks, prior to
operating the overhead hoists.
Among other methods, one feasible and acceptable abatement method is to
ensure that
manufacturer's recommendations for hoist safety practices are followed, in
conjunction
with
safety standards outlined in ANSI B30.16, and that employees receive
formal training in the
operation of overhead hoists to meet the definition of a "qualified"
operator in ANSI B30.16
prior to operating hoists.
