Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)Employees were exposed to the hazard of release of nitrogen at 5,000 psi or greater pressures: 1.On or about December 22, 1996, employees performing seal replacement operations on the nitrogen accumulators; Among other methods, one feasible and acceptable abatement method to correct this condition may include, but is not limited to, installation of a pressure reading gauge on each individual pressure vessel, and installation of a pressure relief valve on each individual pressure vessel in accordance with ASME's Pressure Vessel Code, Section VIII. 2.On or about December 20, 1996, employees performing nitrogen purging operations in the sub-basement area below the nitrogen accumulators; Among other methods, one feasible and acceptable abatement method to correct this condition may include, but is not limited to conformance with ASME's Pressure Vessel Code, Section VIII, Paragraph UG-134. 3.On or about December 20, 1996, employees performing routine production operations around the hydraulic forging presses; Among other methods, one feasible and acceptable abatement method to correct this condition may include, but is not limited to conformance with ASME's Pressure Vessel Code, Section VIII, Paragraph UG-134. 4. On or about December 20, 1996, employees performing maintenance operations in the sub-basement area below the nitrogen accumulators; Among other methods, one feasible and acceptable abatement method to correct this condition may include, but is not limited to conformance with ASME's Pressure Vessel Code, Section VIII, Paragraph UG-135(g) which requires discharge lines from pressure relieving devices to lead to a safe place of discharge, remote from employee exposure.
