Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)Employee(s) were exposed to the hazard of being struck by the bead loosened arm of the Coats 5060 E Rim Clamp tire changer. 1.Employees had operated the unit with the air hose cut away from the attachment end of the broken BL Handle/valve assembly and where the bead loosened handle button was missing/broken off. Among other methods, one feasible and acceptable abatement method to correct this condition may include, but is not limited to conformance with the Coats Rim Clamp Tire Changer Installation, Operating, Safety and Maintenance Instructions that requires regular inspections and proper maintenance by checking for worn, damaged or missing parts including grips and protective covers. Replace them before allowing the unit to be used. On a daily basis, inspect the unit and check to be certain that all systems are operating normally. *** Certification and Documentation of Abatement Required ***
