Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
a)  Mark G. Ettess Arena, northeast section stage: Employees were using a
wire rope
ladder which did not fully extend to ground level in order to access the
theatrical lighting
truss. A wire rope ladder and a 12 foot step ladder were improperly used
in conjunction
with one another to gain access to and from the theatrical lighting truss
on 03/23/97. An
employee fell while descending the wire rope ladder and sustained multiple
injuries.
Employee was exposed to a fall hazard of up to 30 feet on or about
03/23/97.
Among others, one feasible and acceptable methods of abatement is:
1) A ladder should be provided that extends the full height of the
elevation from
the floor to the theatrical truss lighting so that employees would not be
subject to
fall hazards arising from the break between ladders nor the lack of
uniformity of
spacing between rungs of the two separate ladders.
2) Install an overhead access walkway to and from the theatrical lighting
truss.
ABATEMENT NOTE: In Accordance with 29CFR 1903.19, Abatement Verification,
the
employer will furnish certification that the described hazard has been
abated.
Documentation may include but is not limited to photographs, videos, work
orders,
invoices, training records and consultant reports. Abatement documentation
must be
received by the Area Director within 13 calendar days of the final
abatement date for this
violation.
