Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a) Plantwide warehouse areas- Employees were exposed to crushing hazards in warehouse areas as a result of damaged storage racks. Rack columns were bent or torn away from the floor mounts after being hit by forklifts. Storage racks were not protected or maintained as recommended in ANSI Standard MH16.2('84) Sections 1.3, 3.5, and 3.11, on or about 07/30/97. Feasible and acceptable methods to correct this condition as per ANSI standard MH16.2-1984 includes: 1. Installation of isle, frame, or column guards. 2. Replacement or reinforcement of the damaged columns.
