that were causing or likely to cause death or serious physical harm to an
employee in that the
employee was exposed to a crushing hazard:
a)  Loading Dock -
An employee was observed operating a Kamatsu forklift, Model# FB25SH-4,
Type ES,
Serial # 031096A. The employee was not wearing the seat belt which was
provided on the
forklift. On 05-01-98.
NOTE: Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to enforce the use of seat belts by employees while operating
powered industrial
trucks.
ABATEMENT NOTE:  In accordance with 29 CFR 1903.19, Abatement
Verification, the
employer will furnish certification that the described hazard has been
abated.  Documentation
may include but is not limited to photographs, videos, work orders,
invoices, training
records and consultant reports.  Abatement documentation must be received
by the Area
Director within 13 calender days of the final abatement date for this
violation.
