The employer did not furnish employment and a place of employment which
were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to:
On or about 9-05-08:
Location: 1) Hallway by entrance to Public Affairs.
2) In Cubicle/Analyst section.
3) Main entrance/exit door.
a) The employer did not ensure that the work place was kept free of
hazards which
would expose employees to serious injury.
1) An employee was sitting on a chair located on the stairs by the
computer server.
Employee was exposed to the chair slipping of the stairs possibly
resulting in serious
injury.
2) An employee cubicle had books, papers, files, etc. on the floor in the
cubicle.
Exposed employee could possibly slip/trip over said items possibly causing
injury.
3) Employees, entering the building through the main entrance/exit door,
are exposed to
slip, trips and falls by having to step up and into the doorway. The step
up from the
stairway platform is approximately twelve (12) inches.
