employment and a
place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to his employees as the access
midchain on a scissor lift
had not been connected in accordance with the American National Standards
Institute (ANSI)
A92.6 the requirements for self-propelled elevating work platforms.
(a) Trellis area: One (1) employee was performing work from the 15' level
while
standing in an Upright scissor lift, model number 20N, serial number
unknown, which
did not have the access midrail chain connected or other equivalent means
provided (i.e.,
fall arrest system) to prevent a fall hazard.
